1. Privacy policy(Required)

I agree to the Team Mount Beauty Privacy Policy.

2. Risks and hazards(Required)

I am duly aware of the risks and hazards inherent upon entering the event location and participating in the event at this location. I voluntarily elect to enter the location/s and ride in the event. I enter the event at my own risk and responsibility.

3. Helmets(Required)

I agree to wear an Australian Standards approved cycling helmet while riding a bike at all times during practice and the event.

4. Insurances(Required)

I agree that I am responsible for:

* my medical insurance;

* my personal accident insurance;

* my ambulance cover;

* insuring my bike at all times, including whilst my bike is carried upon a TEAM MOUNT BEAUTY INC trailer or vehicle; and

* my own public liability and personal indemnity insurance whilst participating in this event. I understand that my AusCycling membership may provide some or all of this cover.

5. Cancellation of the event(Required)

I acknowledge that the event may be cancelled due to fire, storm, rain, inclement weather, wind, forest closure, or any other circumstances.

6. Rules and directions(Required)

I agree to abide by all the rules and directions of all officials at the event.

7. Loading and unloading bikes(Required)

I understand that I am responsible for installing and removing my bike for carriage upon a TEAM MOUNT BEAUTY INC trailer or vehicle.

8. Indemnity(Required)

I hereby unconditionally indemnify and hold harmless, and shall keep indemnified and held harmless, TEAM MOUNT BEAUTY INC, its committee members, officers, agents, contractors, sponsors and proprietors of any land on which the event or any part of it is conducted from and against all liabilities, claims, suits, demands, expenses, costs, actions and proceedings of any nature whatsoever that may be brought against TEAM MOUNT BEAUTY INC as a result of or in connection with any act, omission, default, failure or error on the part of TEAM MOUNT BEAUTY INC (including any negligent act, omission, default, failure or error) occurring wholly or partly during the event. I agree that TEAM MOUNT BEAUTY INC will not be liable for and I waive any right to claim any loss or damage, personal injury, death, economic loss or consequential loss whether in tort, in contract, under statute (including to the full extent possible under the Trade Practices Act (Cth) and the Fair Trading Act (Vic) or otherwise, for any default, failure, negligence or error on the part of TEAM MOUNT BEAUTY INC. I agree that TEAM MOUNT BEAUTY INC’s liability under any statutory right or any condition or warranty implied by the Fair Trading Act (Vic) or the Trade Practices Act (Cth) or any other relevant legislation which cannot be lawfully excluded is, to the full extent permitted by law, limited to payment by TEAM MOUNT BEAUTY INC of the cost of having the event supplied again.

9. Confirmation(Required)

I certify that I am the Participant and in consideration of TEAM MOUNT BEAUTY INC permitting me to participate in the event, I confirm to and agree with TEAM MOUNT BEAUTY INC and its committee members, officers, employees, volunteers, agents, contractors, sponsors and proprietors of any land on which the event or any part of it is conducted: (1) That I have read and understood the above Assumption of Risk, Declaration, Waiver and Indemnity & Terms and Conditions of Entry and agree to be bound by them; and (2) To give the same release, discharge, waiver and indemnity as set out therein.

9. Confirmation(Required)

I certify that I am the parent or guardian of the Participant ("the minor") and in consideration of TEAM MOUNT BEAUTY INC permitting the minor to participate in the event, I and the minor confirm to and agree with TEAM MOUNT BEAUTY INC and its committee members, officers, employees, volunteers, agents, contractors, sponsors and proprietors of any land on which the event or any part of it is conducted: (1) That I and the minor have read and understood the above Assumption of Risk, Declaration, Waiver and Indemnity & Terms and Conditions of Entry and agree to be bound by them; and (2) To give the same release, discharge, waiver and indemnity as set out therein.
